How this calculation works
A clinic or healthcare provider runs on the same handful of physical inputs as any other site — electricity, gas, refrigerant, water, waste, and staff travel — each converted with one multiplication:
Emissions (kgCO₂e) = quantity used × the factor for that activity
What makes healthcare sites distinctive isn't the method, it's the mix. Cooling load tends to run higher than a typical office because of cold-chain medicine storage and continuous air-conditioning, which is why this calculator gives refrigerant its own field rather than folding it into a generic "other" line. Electricity is Scope 2; gas and refrigerant leaks are Scope 1 (refrigerant is a fugitive emission — the gas itself is a greenhouse gas, leaking directly rather than being burned); water and general waste are Scope 3; and staff commuting is Scope 3 category 7.
Why cooling deserves its own line
Refrigerants used in air-conditioning and refrigeration systems are, gas for gas, far more potent than CO₂. R-410A — the refrigerant in most modern split-unit and cold-room systems — carries a global warming potential of 1,924, meaning a single kilogram that escapes to atmosphere has the same 100-year warming effect as 1,924 kilograms of CO₂. Every AC unit and pharmacy fridge loses a small amount of refrigerant over its service life through fittings, seals, and normal wear, and that loss gets made up during routine servicing — the kilograms added at that top-up are what this calculator asks for.
For a clinic running several split units plus a dedicated vaccine or medicine fridge, even a modest annual top-up can outweigh the site's gas consumption in carbon terms, simply because the GWP multiplier is so large. It's a small number of kilograms with an outsized climate effect, which is exactly the kind of hidden line item a first carbon estimate is meant to surface.

The R-410A factor is published by the US EPA's GHG Emission Factors Hub under public domain terms, drawing on the AR5 100-year global warming potentials used consistently across this site's refrigerant, electricity, and fuel factors.
A worked example
A mid-size clinic with three consulting rooms, a small lab, and a pharmacy fridge tallies a year of records: 8,000 kWh of electricity, 500 kWh of gas for water heating, 1 kg of R-410A added during the annual AC service, 150 m³ of water, 0.5 tonnes of general waste to landfill, and 20,000 km of staff car commuting across the team (four staff driving roughly 5,000 km each per year).
Electricity (Singapore factor): 8,000 × 0.4020 = 3,216.0 kgCO₂e
Gas: 500 × 0.18293 = 91.5 kgCO₂e
Refrigerant: 1 × 1,924.0 = 1,924.0 kgCO₂e
Water: 150 × 0.1913 = 28.7 kgCO₂e
Waste: 0.5 × 520.5327 = 260.3 kgCO₂e
Commuting: 20,000 × 0.16725 = 3,345.0 kgCO₂e

The single kilogram of refrigerant contributes almost as much as the entire electricity bill — a result that surprises most clinics running this calculator for the first time, and exactly why the field exists on its own rather than being buried in a general maintenance category.
What this calculator doesn't cover
- Clinical, biohazard, and pharmaceutical waste. No open-licensed factor exists for these disposal routes in the public registry; keep them out of the general-waste field and track them separately if a disclosure needs the volumes.
- Other refrigerant gases. This calculator asks for R-410A specifically. If your systems use R-32, R-134a, or another blend, use the refrigerant & air-con leaks calculator instead, which covers six gas types.
- Medical equipment embodied emissions. Imaging machines, sterilisers, and diagnostic equipment carry manufacturing emissions before they ever reach the clinic; that belongs in Scope 3 purchased-goods accounting, not here.
- Ambulance or fleet fuel. Diesel or petrol burned in vehicles the practice owns belongs in the fleet fuel calculator, not this one.
- Patient travel. Only staff commuting is counted; patient and visitor travel to the site is a real emissions source but sits outside standard organizational boundary-setting for most healthcare providers.
Where this fits in your reporting
Electricity is Scope 2; gas and the refrigerant top-up are Scope 1; water and general waste are Scope 3; staff commuting is Scope 3 category 7. That split is identical whether the site is a solo GP practice, a multi-site clinic group, or a hospital, and it's what an insurer's sustainability questionnaire, a funder's grant application, or an ISSB IFRS S2 or GRI 305 disclosure will ask for regardless of size. - Why is refrigerant a specific field here rather than an afterthought?
- Healthcare buildings run more cooling than most commercial space — cold-chain storage for medicines and vaccines, always-on air-conditioning for patient comfort, sometimes dedicated cold rooms — and every one of those systems leaks a little refrigerant over its life, which needs periodic top-ups. R-410A alone carries a global warming potential of 1,924, meaning one kilogram of leaked gas is equivalent to almost two tonnes of CO2. For a clinic with several AC units and a pharmacy fridge, refrigerant top-ups can be a meaningfully sized Scope 1 line, not a rounding error.
- Why does clinical waste not have a field here?
- There is no open-licensed emission factor for clinical or biohazard waste streams in the public factor registry this calculator draws from — sharps, pathological waste, and pharmaceutical waste are typically incinerated at high temperature under specific regulatory controls, and the standard commercial-waste landfill and incineration factors don't represent that process. Rather than apply a mismatched number, we leave the field out; track clinical waste volumes separately if you need them for a disclosure.
- Should a single-practitioner clinic bother with this?
- Yes, if you're asked for a number by an insurer, a hospital network, a funder, or simply want to know your own footprint. The arithmetic doesn't change with scale — a solo GP practice and a multi-site hospital group both fill in the same fields, just with very different quantities. Small practices often find electricity and gas alone answer most of the question.
- What counts as general waste versus clinical waste?
- General waste is what any office produces — packaging, paper, food, non-hazardous consumables. Clinical waste is anything contaminated, sharps, or pharmaceutical, and it's handled under separate regulatory disposal routes not covered by this calculator's landfill factor. If in doubt, keep the general-waste field to what would go in an ordinary commercial bin.
- How should staff commuting be measured?
- In kilometres per staff member per year, by car type, entered into the field here.
